You should request a corrected form from the financial institution. Then you can prepare an amended return to see if your tax changes, and if so you can file the amended return. If the financial institution refuses to make the necessary changes you can still amend your return to reflect the accurate numbers, but you should retain documentation supporting your entries in the event you get audited.
