Missing Form SE

Is there a reason why my 2018 filing was missing Form SE even though Schedule C showed income subject to Self Insured taxes? Turbo Tax reviewed the filing and didn't discover this error and I'm wondering why?

How much was your Net Profit on Schedule C? Was it over $400? Did any SE tax show up on 1040 Schedule 4 line 57 which goes to 1040 line 14.

Does anyone know of you can file as married if you are not legally separated from your spouse? Also, there are a lot of medical bills and other documents that are missing
