Do you mean 1040X Part III? (The form itself is only 2 pages).

If so, that's where you explain the changes you are making on the amended return.  Here are some sample wordings for different situations:

(1) Mistyped W-2 form.

(2) Forgot to enter 1099-INT form.

(3) Forgot to claim property taxes paid on Schedule A.

(4) Received corrected 1098 form.

(5) Forgot to indicate that I am a  dependent.
