Refund check

I received a refund check from civista Bank also there was a problem with the check so they've asked me to fill out a form to receive a reprint on the form it asked my tax preparer to sign it now she refuses to sign the form so now what am I supposed to do

Since you had a tax preparer prepare your return, you would need to find out why they won't sign the form.  You can also contact the bank which handles Refund Transfer fees.
