It appears I overcontributed to my HSA by 42.00 in 2018. I received a letter saying I could withdraw that amount by April 15th and avoid a penalty. My question -- Am I required to file an amended return if I filled out the proper form and requested the overpayment back? The letter isn't clear and I'm not a tax pro.

Just get your money back and don't worry about filing an amended return. I doubt it affected your taxes at all.
As long as you request the return of the excess (or applying it to 2019) before the due date of your return, you do not have to amend your 2018 tax return. Keep a record of how you contacted the plan administrator (written request, email or even the date/time you called to request) just in case
