No, it doesn't include the refund of $1,536, although Form 1040X considers your prior refund amount in the calculation.   

You'll owe the amount shown on line 20 of Form 1040X, which, from what you indicate, is $4,712.

No.  The amended return assumes you already received the first refund and shows the difference you get back or need to pay. You can confirm this by printing your amendment and looking at the 1040X. Line 18 should be your original refund amount and line 21 should be your additional refund.  If you paid on your original return it will be on line 16.  If there is an amount owed with the amendment, it will be on line 20.

So you only need to pay the 4,712 from line 20.
