Oregon tax return

Last couple of years the state tax had me separate out medical expenses for me and my wife.  Last year I did not include my medicare insurance expense for me and my wife.  I tried to do an amended return and could not find that form.  I tried to do an amended return several months back and I got an error message when I tried to complete the return.  Confused because now I  try and do an amended return and the form is not available.  Help!
