I omitted a 1099 on the original NJ 1040 tax return. When filing the 1040X, do I have to include the original 1040?
Also, since there was an increase in dividends, should I include the NJ-DOP form? Anything else?

The NJ instructions are somewhat obtuse.
In the case of having e-Filed NJ you attach all your supporting forms to NJ1040-X
such as, NJ-DOP, NJ-COJ, NJ-BUS. even if these don't support your change.
The safest move is to just include everything.
NJ doesn't require you to attach your federal return (1040).
Some Federal forms may support your additional claims.
