Is tax amendment needed? 1099-R in plan conversion.

Hi,

While preparing my tax return for 2023, I found a 1099-R form that I did not file for 2022.

The form covers after-tax 401k in-plan conversion to Roth 401k. Code is G. It does contain $0.03 in box 2a (Taxable amount). It doesn't seem like it changes my tax, but my question is, is it necessary for me to file an amendment? And if so, both federal and state?

No, it is not necessary to file an amended return to report $0.03 in taxable income.  This amount would not materially change your return in any way and it would appear that no changes were actually made with the amended return.

Keep the Form 1099-R with your tax records and make a note that it was not included as part of your 2022 return.
