Yse, and a lady at turbo tax walked me thru it. For 2016 there were 2 different forms, 5 pages total. For 2017 there was just one form, 3 total pages. I think the form changed from one year, but not certain.

Click on FILE then PRINT then save the return and read the filing instructions ... you need to send forms that explain and support the column B entries.
