I have an amended W-2 with a Code DD (and amount) in 12a.. The original W-2 shows a Code C (and amount) & a Code D (and amount) in boxes 12a & 12b. How do I enter this?

Do I enter this as an additional box? Do I remove the first two?

Code DD is for Cost of employer provided health insurance. It is not taxable so it will not change your taxes or income so you do not need to amend. Keep a copy of the W2C for your records.
