I am being asked to fill out form 511ef. I think i may have made a mistake, what’s this form for exactly?

Form 511EF is the Oklahoma Individual Income Tax Declaration for Electronic Filing form.

If your return contains any of the forms or supporting schedules shown in the pic attached (OTC table), you are required to mail the documentation to the OTC.  Mail any items shown in the table below along with the Form 511EF to the address shown at the bottom of this form.

Since I printed the forms from turbo tax, do i have to physically sign them?

You only need to sign in the space(s) provided in Part Two of Form 511-EF, Oklahoma Individual Income Tax Declaration for Electronic Filing.
