That is not a federal requirement.  What state requires a retirement date on the state tax return?

Proof of retirement date is required when requesting waiver of underpayment penalty on Fed Income tax when reason is based on retiring in 2020.  I used the "Annualized Income Method" to lower penalty.  Do I need to submit hard copies of return with proof of retirement?  I don't believe I have efile option.  Thank you.
