An M1LTI is not a general health insurance form; it is the Minnesota Schedule M1LTI (Long-Term Care Insurance Credit), which the tax software is prompting you to fill out because you indicated you are filing a Minnesota state tax return.

What is Schedule M1LTI?

  • It is a state tax credit for people who paid premiums for a qualified long-term care insurance policy.
  • To claim this credit, your policy must qualify as a federal itemized deduction and have a lifetime benefit limit of $100,000 or more.
  • It does not relate to standard employer-provided health insurance, and you do not need a Form 1095-C to fill it out.

What You Should Do in Your Tax Software

  • If you do not have long-term care insurance: You answered "Yes" or clicked through a prompt asking if you had long-term care insurance by mistake. Go back to the state/credit section of your tax software and select No or skip the Long-Term Care Insurance Credit.
  • If you do have long-term care insurance: You need the policy statements and proof of premium payments for that specific long-term care policy—not your Form 1095-C.
  • If you do not want or qualify for the credit: Skip or clear out Schedule M1LTI so your state return can proceed without it.
