Need to file amended return?

After filing my return I realized that form 8949 was incorrect. The adjusted cost basis for some ESPP sales were entered directly in column (e) instead of columns (f) and (g). The gain/loss amounts in column (h) are correct (and my total refund is also therefore correct), but do I need to file an amendment to correct form 8949?

No, I would not file an amendment, unless the actual forms or amounts changed.

I would save that information for 7 years (you should save all information regarding taxes for 7 years) just in case the IRS asks about it.
