The 21 days is a number TurboTax provides as an estimate, since the IRS says that most people who efile and choose direct deposit will get their refunds within 21 days.  Sometimes it's very fast, such as a week or 10 days.  Sometimes it can be much longer than 21 days, depending on how much review and processing the IRS feels like it needs to do.

What is displayed currently when you use the IRS "Where's My Refund" tool?

With the "Up to 5 days early" offer, once the IRS releases your refund if you don't get it at least 1 day earlier than you would have if the IRS had sent it to you directly, then TurboTax says they do not charge the fee for the that offer.   Some filers who chose that offer don't have a fee anyway if they chose to get it in a Credit Karma MoneySpend account instead of their own bank account.
