How do I include form 8283 with my form 1040 for donation of stock above $5,000?

TTX will create one.

You enter the donation to the charity

Name of Charity

Select "Items & Stock" donation.

Then Type of Donation sub-select "Stock....."

Date of Donation

Donation Value.

A page or two later the  Stock Symbol, #of shares, valuation method   (TTX uses comparative sales...others are complaining about this, but I've used that for 10yrs with no problems)

 

And continue thru the rest of the questions

 

You will need the address of the DAF.

But keep a copy of the actual 8283 that the charity should-have sent to you.....in case the IRS has any questions later....that can happen even if entered properly.

To include Form 8283 for stock donations over $5,000 in TurboTax, you will have to enter the information in the Deductions & Credits- Charitable Donations section, and select "Stocks" as the donation type and input the required appraisal, cost basis, and acquisition date

 

TurboTax will create the 8283 to include with your tax return filing.
