I efiled my state of CO return. Do I need to file a separate DR0104 form to get the TABOR refund? The information on the state website is unclear.

Yes, you must file a DR104 to get the TABOR refund, but it should be on your Colorado return already.

Check line 34 of your DR104 to make sure that you claimed the TABOR refund with your state tax return. You should see $800, or $1,600 if a joint return, on line 34.

If line 34 is blank and you are a full year resident and at least 18 on December 31, 2022, you are eligible to get the TABOR refund. You can amend your 2023 DR 104, or file a 2023 PTC to get your refund.

Here's how to amend your state return:

Sign in to your TurboTax account.
Select Add State.
Select State Taxes, then Your State Returns.
Select Edit to the right of Colorado.
On the screen, Your 2023 Colorado taxes are ready for us to check, select Edit to the right of Other situations.
Select Start to the right of Amend a Tax Return.
Follow the on-screen directions to complete your state amendment.

Do I have to upload the form separately, if I e-files my CO return using TurboTax? This is for 2024 return (e-filed on April 6, 2025). Thank you

The TABOR refund will be factored into your annual tax burden, meaning if you're getting a Colorado tax refund, you'll get even more. If you owe tax to the state, then you'll owe less. The amount of Tabor refund is reported on your DR 0104, Colorado Individual Income Tax Return Line 35.

Open your CO state return and continue until you see a screen with the title, Qualifying for the Colorado Sales Tax Refund. Answer the next two questions and the amount will be included on your return.

I emailed my Colorado Tax return. I did not complete line 34. Turbo Tax did ask me to make entries. I sent the return to the State of Colorado.

As noted by @JulieS you can amend your CO state return (steps extracted below). Once you have started the amend process (step 7 below) you will need to navigate back to the "Other Situations" page (step 5 below) and find the "Sales Tax Refund" topic to claim the rebate.
