I received a 1099-MISC for royalties on a book published 30 years ago. How do I answer the question on "materially participate" in the operation of this business in 2018? I haven't done anything on the book since it was published.

Yes, you need to say you materially participate as you wrote the book. The 1099-MISC should be entered into the Rental and Royalties section of TurboTax.
