Since no 'total' on the form appears, is it OK to enter each line as a separate 1099-R form? Or is it more appropriate to total the amounts on the one form? The only changes on each row would be the amounts for distribution, federal/state tax.

If box 7 distribution code is the same for all of the distributions, you can total and enter it all as one 1099R.
The total amounts is what shows on the tax return and what the IRS matches the 1099R to the return with.
Just carefully total the amounts so all are entered.
If some amount would have a different distribution code in box 7 that would need to be entered separately.
