Box 1 and Box 2a are the same amount. Box 2b is blank.

And I don't believe there were any earnings. So it should all have been a return of an excess 2018 contribution.

I appreciate your response!My issue is that the administrator for the 401(k) has incorrectly issued the 1099-R with a code "8" rather than the correct code "P"....
