Form 8960 works by going through the calculation if your modified AGI triggers that it might be applicable. The good news is that when the form becomes available and you are able to file your return, you will not incur a delay in processing since the IRS will not start processing returns until February 12, 2021.
To read more about Form 8960, take a look at the following TurboTax article.
What is Form 8960: Net Investment Income Tax

Even though the Form 1040 was not the main form that was changed, there were items on that form that did change as a result of your amended return. You should include the Form 1040 and Schedule B with your Form 1040X.
In the long run, it is better to include the form with the amended return instead of risking a delay in processing because it was not included.

Take a look at the information that has been entered in boxes 10a, 10b, and 11. If anything was entered in any of those boxes, then they all must be filled in.
If you did not have any state taxes withheld, then leave all the boxes blank. This includes the dropdown box for the state name.
If you did have state taxes withheld, but there is not a state ID number on your form, then use the Federal ID number in box 10b.
Following these steps should eliminate the need to review the entry again.

Since you did have state taxes withheld, that means that boxes 10a, 10b, and 11 must be completed to avoid an error condition.
For box 10b, you can use the Federal ID number since no state ID number was provided on your form.

If you are working through the business (Schedule C) section of your tax return, the question is asking about the income of the company instead of your income personally.
You will enter all of the income and expenses of the company into the business section of your return. Then, this information will be reported on Schedule C. The bottom line profit or loss from the company is then included as part of your personal tax return.
If you have an overall profit, then you have essentially added to your personal income even though you never paid yourself from the company. If you have an overall loss, then that will be taken into account to reduce your other taxable income that is included as part of your return.

It is unlikely to be rejected by the electronic filing process. Instead, farther down the road the IRS would examine the return and match documents during processing and the error could be caught at that time.
At this point, you should wait to see if your return is accepted or rejected for another reason. Then, if it is accepted, you can amend it to make the correction and remove the 2019 W-2.
Keep the following TurboTax article handy for reference. Amending instructions will be available a bit later in the tax season after the IRS has started processing returns.

The information you shared is an electronic version of Form 1099-G that reports the unemployment that you received in 2020.
When you are working on your tax return and entering information for Form 1099-G, you will enter the total amount of unemployment compensation you received and the Federal taxes that were withheld. Since there were no state taxes withheld, you will not need to enter those details.
When you are ready to enter the information about your unemployment, use the following steps in TurboTax:
On the top row of the TurboTax online screen, click on Search (or for CD/downloaded TurboTax locate the search box in the upper right corner)
This opens a box where you can type in “ unemployment ” (be sure to enter exactly as shown here) and click the magnifying glass (or for CD/downloaded TurboTax, click Find )
The search results will give you an option to “ Jump to unemployment ”
Click on the blue “ Jump to unemployment ” link

Try going back to the 1099-G entry screen and be sure that there is nothing entered in the state dropdown box (10a) or in either box 10b or 11. Scroll all the way to the top of the drop down choices and choose the top option.
Since Texas does not have a state income tax, no state taxes should have been withheld. Therefore, all three of those boxes (10a, 10b, and 11) must be blank in order to avoid an error condition.
