Hello and thanks for joining us today!
From my understanding, based on the information provided on the Texas Workforce Commission website, you may file an appeal if you have been denied unemployment benefits and/or were asked to pay back the unemployment benefits received.
If you did not receive any unemployment benefits, this might not apply to you and the form provided is only informational.
If you would like to be certain, please feel free to contact us so that an expert can take a look at your tax return and review this with you.

Thank you @rain89632 for your question. I would suggest that you contact the Texas Workforce Commission at 800-628-5115. They should be able to tell you why there is a document relating to an action that they took.
Be well and safe.
All the best,
Marc T.
