Are you sure it's not included in your Box 1 W-2 Wages. I would be leery claiming it without claiming Federal Withheld. You don't want to pay more tax than you have to. At a minimum I would report it under "other income"

My husband does not have a W-2 or access to any paystubs from his previous employer. The company filed bankruptcy & there's no one to contact to obtain his W-2. Now what?
