Try closing TurboTax Online, clear your Cache and Cookies and then open TurboTax and delete the Disaster Worksheet.
Then return to the interview section for Form 1099-R and go through and answer the Disaster Questions again to re-create the worksheet correctly.
I finally got it to delete and the form to process correctly but what a pain in the rear. First I had to delete that I ever received a disaster distribution and go all the way through section with it that way to get the Form removed. I went through the process like you described in it filled out the form wrong again. Then I did the process again to remove the form. The I went through the section again the way you described except I didn't go back to check the Corona virus box but it still put Corona virus on the form in Item C even with the box in Item D unchecked. Finally got it to work by going through the 1099-R section again but selected a different disaster and completed the section, then went back in and removed that disaster and then paged back and selected the Corona virus box and completed the section. When I went to review the forms it finally wasn't in Item C and only showing in Item D.

DELETING the checked box "Did you ..take qualified disaster relief".. (or whatever) on form 8915-E in the forms menu WORKED !!!
It finally got rid of Form 8915F-T which I tried to delete MULTIPLE times (and it didn't go away!)
