What if I don't have a 1099-HC

Were you a MA resident during 2023?  Every Commonwealth of Massachusetts resident who has health insurance will receive a 1099-HC form. This form is provided by your health insurance carrier and not the Group Insurance Commission (GIC).   To download an electronic copy of your Form 1099-HC, you may log in to your medical insurance carrier’s online account, or contact their member services at the number on your ID card for more information.

If you can’t find your 1099-HC from Mass Health, you can call them at (866) 682-6745; TDD/TTY: 711 for a replacement copy.  Here is the link in case the phone number gets deleted...
