The 12 months check box on the Personal Information Healthcare Coverage worksheet will not accept a checkmark? Is it ok to just check off each of the 12 monthly boxes?

Yes - having all 12 months checked off in the monthly boxes instead of the 12 months checkbox is actually the way that worksheet was designed when you answer the question ("Yes" to health coverage all year) in the health insurance interview section. That checkbox is an override option when using the Forms mode in desktop. So, it can definitely cause some visual confusion when looking at that worksheet, but as long as all your months show checked in the section below that is what the program used to calculate your return.
