You will need to contact that prior employer to ask about a Form 1099-R. If it is a true distribution from a "retirement fund" you should have received that form. Keep in mind, many employers and fund managers are no longer mailing forms and you may need to obtain it online. Either way, contacting the former employer will give you the information you need to obtain the 1099-R. Once you have that, you can enter it into the software in the Retirement income area.
