I've just filed an extension with the wrong information and I need to send a payment, Can I submit a new extension or should I mail a new extension form with the check?

if you mail funds separately after filing 4868 electronically, use a filled out form 4868 as a voucher.

Also indicate that your request for extension was already submitted electronically.
