The Medicare and Supplement insurance are entered when you enter that spouse's SSA-1099 from Social Security. You do not enter that anywhere else.  Affordable care insurance premiums are entered as medical expenses on Schedule A of your return, under the Deductions & Credits/Medical Expenses section of your return. 

Under the Health Insurance section of your return, you will respond to the question about your health insurance coverage that by selecting the appropriate option (see screenshot below)
