Non qualified annuity withdrawal

Put in 100,000 in nq annuity took 50,000 withdrawal net 42,000 because of 10% early withdrawal and surrender charge

My 1099r taxable amount blank tax not determined checked codes are D AND When I enter 1099r in turbo tax says I still have to pay 10% early withdrawal plus taxes on 42000 money I already paid taxes on I had requested 20% be withheld just in case for tax

Called financial institute said there where no earnings or interest to taxes but why than is taxable amount blank they give me no answer
