Is there a reporting error on W-2?

box 1 wages is empty

box 2 Fed withheld is empty but box 3 & 5 have amounts

Live in California, which has a state tax, but no information is listed for boxes 15, 16, 17.

No to home health care, worked as a union trademan

Yes, I was a care giver to my mother.

If Box 1 and Box 2 are both empty on on W-2, do not enter it into your IRS return.
