Medicare premiums on both RRB-199 and RRB-1099-R forms

The boxes shown for the RRB-1099-R are incorrect. There is no line 12 on the form which asks for Medicare premiums. The RRB-1099 is the form that shows the Medicare premiums and it is in box 11. This is so confusing because it is unclear where exactly TT wants us to input the number. Only input the Medicare premiums on form RRB-1099 or you will have duplicate deductions.

I think you may be entering your form in the wrong spot.   You probably need to delete that entry.  The RRB-1099R gets entered on the next line, with the 1099-R forms.   You should choose the last option in the list of 4 types of 1099-Rs.    The RRB-1099 and the RRB-1099-R are entered in 2 different places.
