Late Corrected W2, Multi state 4852

I received a W2 from my prior employer back in January. The W2 needed a large correction (>10k) and my employer has been very slow processing that correction. I've asked them about it several times and it's still being processed. Recently I've contacted the IRS to send them a letter asking for my corrected w2.

If I don't get my corrected w2 soon my main option looks like file 4852. Looking at the 4852 form I see a slot for state taxes withheld. What do I put there if there were multiple state taxes withheld and later for that company? I only lived in one state but they incorrectly withheld in a different state for a few months until they eventually fixed it and used my actual state. Do I put the sum of the two state taxes withheld and write name of both states? File 2 4852's and mostly duplicate numbers?
