It keeps pulling up the K-40SVR form and asking me to fill in the appraised property value. I do not know how to remove this form from my file that doesn't apply to me

The form says its property tax relief claim for seniors and disabled veteran. I'm not either of those and I have went back and made sure those questions were answered with no. I don't know why this form is still on my file. I need help removing it
