If I was on my dad's health insurance all of 2016, what do I answer for 'What type of High Deductible Health Plan did you have on December 1, 2016'? Family or none?

You would answer "none" to that question in this scenario since you do not have your own separate HDHP. If you answer "family" you will get a notice about a coverage lapse.
