If I mail my massachusetts return, what do I need to send along with the printed tax return pages?

You should have mailing instructions (including what documents to send) when you print out your return to mail.

Be sure to include all four pages of Form 1, as well as Schedule HC and all other applicable schedules.

Enclose with your Form 1 all state copies of your Forms W-2, W-2G, PWH-WA, 2G and any Forms 1099 which included Massachusetts withholding.
