I was on short term disability last year for 4 weeks. It was paid through EDD. What tax form do I need?

I was on medical leave of absence for a surgery and was off work for 6 weeks. I was paid through EDD. Is there a separate form that I need for taxes?

Assuming you are in CA. Generally, disability payments aren't taxable that you receive from EDD, but if they are, you would receive a 1099-G.
