I recently recieved my W2 which included my paid family leave SDI. Will I receive another form for the additional SDI for the time I was out of work beyond the paid family leave?

You may. There are many types of disability payments/insurance, some taxable and some nontaxable. It's best to contact the plan administrator to see if you will be receiving another form.
