I changed my last name in 2018. for 2017 tax filing. Do I need to use my new or last name now? I mean on information page? or keep using the same information on my W2?

You will need the last name that the Social Security Administration currently has on file. The IRS matches the name and SSN on your return with the Social Security Database.
