Yes, you should file an extension. If you expect that you will owe when you file, you can make a payment with your extension. If you expect a refund, you don't need to make a payment.
You can file an extension through your TurboTax account, or Easy Extension here.
To get your return done, you need to contact your former employer and get a copy of your W-2 form. If they refuse to make it available, you can ask the IRS to help you get your W-2 form. Follow this link for more about that.
