How to handle an American married to a Canadian w/o SSN?

We have filed an extension. A married couple, US Citizen and a Canadian citizen (applying for a green card). We hoped the ITIN number would have been received by now, but it hasn't. How do we file? Does it have to be married filed separately? Is there a way to still filed married without the spouse having a social security number or an ITIN?
