A w-2 is a Wage and Tax Statement. Were you an employee or were you self-employed? If you were an employee, you need a w-2. If you received a Wage and Tax Statement, it should have all the necessary information to enter the w-2.

If you were self-employed, use the instructions linked here for cash payments. The income will go on Schedule C; you don't need any tax forms.

For the earnings from Treasury Direct, I selected box 3 for the tax return. I noted that there was an increase on California State Tax. Should it be exempted?
