How do you report income and federal income withholdings from Massachusetts Paid family medical leave on form 1099-G?

I received a form 1099-G for income received from the Massachusetts Paid Family Medical Leave (PFML) with the Federal income tax withheld amount in line 4.  How do I enter the income and withholdings  as it does not apply to the options given, nor does it give me a field to enter amounts in box 4.

How do you report income and federal income withholdings from Massachusetts Paid family medical leave on form 1099-G?

Make sure you are entering the 1099-G in the right place. Paid family medical leave is entered the same as unemployment benefits. In the Search box type 1099-g, then click the link that says "Jump to 1099-g."


On the screen where you enter the amount that you received, below where you select the state, check the box that says "Federal or state income tax was withheld on this payment." Then when you click Continue, the next screen will give you a place to enter the tax withheld in box 4.
