How do I get a copy of my 1095-a form if the market place website isn't working

You should have received your Form 1095-A no later than mid-February, but only if you purchased health insurance in 2016 through Healthcare.gov or your state’s Health Insurance Marketplace.

If you didn't receive your 1095-A or lost it and cannot get a copy from your Marketplace online account. You can contact them directly for assistance.

Below you will find an FAQ with a list of the states with their own Health Insurance Marketplaces. If your state is not listed, use Healthcare.gov or call 800-318-2596.

Yes I have tried very hard to get information like a PDF file or an email of my 1095 from 2019
