You must file form FTB 3849 to reconcile any PAS (Premium Assistance Subsidy) advanced to you with the actual allowed PAS amount based on your actual household income for the applicable taxable year, less federal Premium Tax Credit (PTC) amounts.
If you bought health insurance through one of the Health Care Exchanges, also known as Marketplaces, you will receive Form 1095-A Health Insurance Marketplace Statement and, Covered California will send Form FTB 3895 California Health Insurance Marketplace Statement to all enrolled members.
Report form 1095-A on your return and when you get to your state-filing, you will be prompted to enter Form 3895 on your CA tax return. Once you enter form 3895, TurboTax will generate form 3849.
To add the 1095-A form :
- Select Federal from the left menu.
- Click on Deductions and Credits
- Click on I'll choose what I work on (if shown)
- Scroll down to Medical
- On Affordable Care Act (Form 1095-A), click the start or update button
