For Ohio estimated payments Turbotax forms do not show the quarter or due date. How do I determine what quarter pertains to which form? Only 3 forms are printed.

The actual estimated payment forms do not have the quarter listed for Ohio. When you prepared your tax return, there should have been a page with instructions that also printed out with directions on how much to pay along with the due date of each payment.

I recommend logging back into your TurboTax account and reviewing the pdf copy for these instructions. Please see this link for guidance on how to view or print your pdf copy. For Ohio, you will be looking for filing instructions for Form OUPC.
