For my state taxes it keeps asking me to fill out a sch in-occ form, i have never heard of such a thing and havent recieved one with my w2, should i call my employer?

it lists a number in the 400s in the auto-filled out portion they are asking me for on turbo tax & it mentions scholarships, i am not a student. i am a part time employee with no benefits.
