Firm 8915

Filed in 2017 for a disaster relief from my IRA withdrawal due to Hurricane Harvey. 
1/3 claimed in 2017, 2018 and 2019. Why is it showing up again in 2020.

Some people received a qualified 2017 distribution in 2018, which obviously changes the years the form is needed to 2018, 2019, and 2020, but still begins with the 2017 disaster. Since yours was 2017 distribution in 2017, you do not need the 8915 and can delete the form. See How to Delete
