Excess removal from roth ira for 2019

It looks like they didn't issue a 1099-R for that additional $130 distribution. I would contact them regarding the error. If they refuse, you can create a substitute 1099 using all the same information on the original 1099-R except in Box 1 and 2A, you will report $130. if you use Turbo Tax online

  1. After entering the 1099-R, there is a screen that asks Do any of these situations apply to you?
  2. You will check the Box I need to file a substitute 1099-R
  3. Then Turbo Tax will ask why you had to file, here you will indicate that this is a return of an excess contribution but the payer refused to issue a 1099-R. 
  4. Ask the payer first if they will issue a 1099-R for that additional $130.

They would not issue 1099R  as $130 was not distributed eventually. (- and then +. Basically put 130 back to Roth IRA. I have no idea why it happened. 
I need to call them tomorrow.

So I spoke to Vanguard and after 1.5 hours waiting, they admitted it was error and they will send me corrected 1099 R with amount 6000 as distribution. . It will take couple of days.  
My question now is that the taxable amount in corrected 1099-r  should be 0 instead of “not determined” as there should be no tax or penalty for removal of principle. Am I correct?

Correct, the removal of a Roth contribution is tax- and penalty-free.
